TIER TWO: COMMUNITY-BASED OUTREACH
Health2U™ Mobile Health Clinics
Overcoming the healthcare access barrier for West Georgians in the most urgent need of care.
Can you imagine
equitable, accessible healthcare for all
?
Our Community's Need
Lack of healthcare access is one of the most significant barriers West Georgians face, and is directly related to poor outcomes. As many as 90,000 Georgians in Carroll, Douglas, and Haralson counties may unknowingly show early indicators of preventable chronic heart disease.
Example: Carroll County, Georgia
-
14,000 annual emergency room visits by uninsured individuals
-
Most common complaint: abdominal and chest pain; many of these individuals have multiple comorbid conditions.
-
Hospital system cost: $133M
THE CHRONIC DISEASE CYCLE
DISPROPORTIONATELY AFFECTS THOSE LIVING IN POVERTY FOR GENERATIONS
THE CHRONIC DISEASE CYCLE
IS BROKEN BY THE INTERVENTION OF ON-DEMAND, ACCESSIBLE MEDICAL SERVICES

Breaking the Cycle
Through our research, we have identified individuals and areas of greatest need according to ZIP code, and can provide healthcare access directly to those communities .
Our mobile clinic van will be fully equipped with two exam rooms to address acute needs, as well as chronic illnesses at no cost to the participants.
Over a five-year period, we expect our early detection and prevention strategies to reduce hospitalizations and address the comorbidities associated with the number one killer of adult men and women.
Reimagining Healthcare
Health2U™ Mobile Health Clinics will help:

-
decrease morbidity
-
decrease mortality
-
decrease healthcare costs, and
-
increase workforce productivity

in our three-county region.

We have established community-based clinic sites in which we obtain data on blood pressure, diabetes, cholesterol, and BMI, and refer affected individuals to various clinics for follow up. These free screenings are performed every six months at select sites, and help empower various institutions in our communities to become ambassadors and sub healthcare clinics.
FAMILY GENERATIONS CAN BE TRANSFORMED WITH EQUITABLE, HIGH-QUALITY HEALTHCARE
A new future is within reach.

Those of us with the resources to do so have a great calling upon us to serve our communities by removing obstacles and barriers for hard-working families who have been underserved for too long. Together, we can foster an entirely new mindset for healthcare in our communities, creating new family legacies, and impacting generations to come.
